Globally it is estimated that 347 million people are living with diabetes and that half of them have not been diagnosed. Today, type 2 diabetes is growing exponentially due to the global obesity epidemic; and both conditions threaten to exceed available health care resources.

The good news is that studies indicate that type 2 diabetes is preventable. The incidence of this disease can be reduced by up to 50% with changes in lifestyle and pharmacological interventions, but a highly relevant fact is that bariatric surgery reduces the long term incidence of diabetes by up to 80%.

A revealing study conducted by Swedish researchers at the University of Gothenburg and published by the Journal of the American Medical Association, found that in the long run, obese patients with type 2 diabetes who underwent bariatric surgery have higher remission rates and fewer complications than patients undergoing usual care.

The Swedish study followed 343 patients that had undergone bariatric surgery and compared their results with those of 260 patients that were not operated on. After two years, 72% of the bariatric patients remained in remission from diabetes; while only 16% of the other patients avoided the disease. 15 years later, 30% of the bariatric patients sustained remission, compared to 7% of the other patients. It is worth mentioning that the study also found a reduction in micro and macro vascular complications in the bariatric patients, which resulted in fewer strokes, heart attacks and peripheral vascular complications, as well as a reduction in neuropathy, retinopathy and kidney problems. This type of data has led a growing number of experts to conclude that the most effective method for treating this serious situation is in fact metabolic surgery.

Bariatric surgery’s non weight-reduction related mechanisms responsible for diabetes remission are attributed in part to hormonal changes that occur in the digestive system after surgery. In 2013 diabetologists, endocrinologists, surgeons and public health experts gathered in New York for the 2nd World Congress on Interventional Therapies for Type 2 Diabetes. Based on data presented there, the International Diabetes Foundation made public its position on the need to consider bariatric surgery as a viable option in the early treatment of diabetes. In their statement, they recognized the following:

– In addition to behavioral and medical treatments, bariatric surgery is a powerful option to reduce diabetes in obese patients.

– Bariatric surgery is a suitable treatment for people with type 2 diabetes and obesity who have not achieved improvement through conventional medical therapies.

– Surgery should be an acceptable option for people with type 2 diabetes and a BMI above 35.

– Surgery should be considered as alternative treatment in patients with a BMI between 30 and 35 and when diabetes cannot be controlled through an optimal medical protocol, especially in the presence of other cardiovascular risk factors.

– Bariatric surgery as a cost efficient treatment for type 2 diabetes.

– The risk of complications or mortality due to bariatric surgery is low and is similar to that of well accepted procedures such as gallbladder surgery.

– Metabolic and bariatric surgery for the treatment of type 2 diabetes should be conducted under strict guidelines, which include multidisciplinary pre and post surgical care, patient education, clinical follow-up and of course, safe and effective surgical procedures.

At LIMARP we are completely up to date on the progress and benefits of bariatric surgery; our comprehensive care model for the obese patient is at a level of global excellence and it addresses the problem of obesity and type 2 diabetes in a safe and accurate way, in an increasingly large number of patients.
